  • Form 1099-R for pension/IRA/annuity distributions (including Roth conversions)
  • Form SSA-1099 for Social Security Benefits
  • Form RRB-1099(-R) for independent retirement program contributions administered by the RRB (Railroad Retirement Board)
UNEMPLOYMENT INCOME
  • Form 1099-G for unemployment benefits (For this form you will have to go online to your nystax.gov account and download the pdf, they will NOT mail you a copy of this form)
SAVINGS, INVESTMENTS, & DIVIDENDS
  • Form 1099-INT for interest income from your bank
  • Form 1099-OID for Original Issue Discount income
  • Form 1099-DIV for dividend income & other distributions reported by financial institutions
  • Form 1099-B for proceeds from broker and barter exchange transactions
  • Dates of acquisition & records of your cost or basis in property you sold (if basis is not reported on 1099-B)
  • Form 1099 Composite for a consolidated list of the above forms
  • Expenses related to investments
  • Form 1099-SA for distributions made from a health savings account (HSA), Archer medical savings account (Archer MSA), or Medicare advantage medical savings account (MA MSA)
  • Form 1099-LTC for Long-term care benefits, including accelerated death benefits
  • Form 1099-S for proceeds from the sale or exchange of real estate
  • Form 1099-MISC for royalty income
SELF-EMPLOYED INCOME
  • Form 1099-K for income from a payment card or third-party network transactions
  • Form 1099-NEC for any non-employee compensation
  • Form 1099-MISC for any miscellaneous income received
  • Any other income not reported on forms 1099
  • Records of all expenses including check registers, credit card or bank statements, and receipts
  • Any business use asset information including the cost and date placed in service
  • Home Office information including the square footage of your office and your house

  • Receipts for any qualified educational expenses
  • Form 1098-E for any student loan interest paid
RETIREMENT & OTHER SAVINGS
  • Form 5498-SA for any HSA contributions
  • Form 5498 for any IRA contributions
  • Form 5498-ESA for any Coverdell ESA contributions
CHARITABLE DONATIONS
  • Any receipts for donations to any qualified charitable organization
  • Records of non-cash charitable donations
  • Records of miles driven for charitable purposes
MEDICAL EXPENSES
  • Any amounts paid for healthcare, insurance, and to doctors, dentists, and hospitals
  • Records of miles driven for medical purposes
CHILDCARE EXPENSES
  • Fees paid to a licensed daycare center for care of an infant or preschooler
  • Expenses paid through a dependent care flexible spending account at work
HEALTH INSURANCE
  • Form 1095-A if you’re enrolled in an insurance plan through the Marketplace or Exchange
  • Form 1095-B if you’re covered by minimum essential health coverage
  • Form 1095-C if you’re enrolled in an employer-provided health insurance plan
TEACHER'S (K-12)
  • Any receipts for classroom expenses
OTHER
  • Form 1040-ES for any estimated tax payments you made
  • Any invoices showing vehicle sales tax paid or personal property tax on vehicles
